A straightforward process from transcript request to delivery.

01 · Request

Complete the official transcript order form

Download and complete the official Transcript Order Form of an Ontario Court Proceeding. Sections 1 through 5 are completed by the ordering party.

The form asks for

  • Case and court information
  • Type of proceeding
  • The portion or content to be transcribed
  • Transcript format and requested service level
  • Date the transcript is required
  • Ordering-party contact information

Section 6 is completed by the Authorized Court Transcriptionist.

Please do not send court recordings, transcripts, exhibits or other sensitive materials through the public website.

02 · Confirmation

Send the completed form and confirm the order

Send the completed order form to RecordVerity. The request will be reviewed to confirm the requested scope, availability, anticipated cost, turnaround and any additional information required before the assignment is accepted.

  • Delivery arrangements
  • Any additional information required

03 · Recording

Obtain the required recording and materials

For Ontario court transcripts, the authorized recording and related materials are obtained through the applicable court process after the order arrangements are confirmed.

Certain proceedings may be subject to access restrictions or additional authorization. Where another step is required, RecordVerity will identify it before transcription proceeds.

04 · Preparation

Preparation and review

The transcript is prepared and reviewed with attention to:

05 · Delivery

Electronic delivery

The completed transcript is delivered electronically according to the agreed arrangements.
